- Schedule your consultation by clicking here or by calling (855) 830-2020
- Are you already enrolled for 2014? Be sure to use the funds that you have set aside before they expire on 12/31. In other words, use it or lose it!
- Need to enroll for 2015? Check with your HR Manager to find out if a Flex Spending Account or Healthcare Savings Account is available and when the enrollment period is. When you come in for your consultation, you’ll already know if you’re a candidate for LASIK and can then determine how much to allocate for your procedure.
- When it’s time for your LASIK procedure, use your Flex Plan card to pay or submit your receipt for reimbursement. Once you enroll, your plan is effective immediately you don’t have to wait until the end of the year to use it.
